Passport services are not available on weekends or on Zimbabwe or U.S. holidays.
Applicants Age 15 or Under Without Both Parents Present
The DS-3053 is the Statement of Consent for Issuing a Passport to a Child and the DS-5525 is the Statement of Special Family Circumstances.
Adult Passport Renewal (DS-82)
Renew an existing passport issued to you when you were aged 16 or older.
Report and Replace Lost/Stolen Passport (DS-64 & DS-11)
Report a passport lost or stolen and replace it.
You can fill out the correct form, or find information about your passport status at https://travel.state.gov.
You must provide one photo with your passport application. All our photo policies apply to both adults and children under age 16. We have a few exceptions to our photo policies for infants.
The Consular Section is available to assist American Citizens with emergency passport applications at any time during normal working hours. For passport applications, please bring the following: Most recent passport, 5cmx5cm photo (note: this is NOT the standard size Zimbabwean photo), Cash or Credit Card.
